All plans are living documents, reflecting who the person is today, knowing that what they may want tomorrow could vary. Support providers must remain committed to being flexible and engage in ongoing dialogue using the plan as a guide (or north star). We are all accountable to the person’s plan and must work together to support the person in striving to achieve their desired goals.
Planning is not a one-time event to meet policy requirements; it is reflected in our day-to-day actions and support of people.

Developed by Helen Sanderson and Associates, this approach takes the principles of person-directed planning and looks at what it means in practice as a service organization. Person-directed reviews result in person- directed outcomes that can be used to shape the (individual support) plan and how services are delivered in the future.
